설사 기전과 설사의 원인, The mechanism and causes of diarrhea

 

설사 기전

  1. 장관(창자) 내 장액(Intestinal fluid)의 삼투압이 정상 이상으로 높을 때 장관 벽 점막층을 통과해 혈관 내 수액과 조직 속 체액이 장관 속으로 정상 이상으로 많이 분비되어 생길 수 있는 설사를 삼투압 설사라 하고
  2. 흥분성 위장증후군이 있을 때와 같이 장관 연동이 정상 이상으로 항진되어 생길 수 있는 장관 연동 항진성 설사 
  3. 바이러스 위장염이나 박테리아 위장염 등으로 장관 벽 점막층이 손상되어 그로 인해서 혈액 속 체액이 장관 속으로 비정상적으로 많이 분비될 수 있다. 또 장관 속 장액이 정상적으로 장관 점막층의 혈관 속으로 흡수되지 않을 때 생기는 설사를 분비성 설사라고 하고 
  4. 궤양성 대장염이나 크론병 등 염증성 장 질환으로 장관 벽 점막층에 염증이 생겨 그로 인해서 장관 내 장액이 증가되고 핏속의 체액이 장관 속으로 정상 이상으로 분비되어 그로 인해서 설사를 할 때 염증성 설사라고 한다.
  5. 위에서 설명한 것과 같이 이론적으로는 각 종류의 설사의 기전으로 인해서 한 종류의 설사도 할 수 있지만 대개의 경우, 설사는 여러 가지의 기전이 동시 생기고 그로 인해 설사를 하는 것이 보통이다. 이렇게 하는 설사를 혼합형 설사라고 한다.
  • 이미 설명했지만, 이론상으로, 설사가 생기는 기전에 따라
  • 삼투압 설사, 
  •  분비성 설사, 
  • 장괸 연동 항진성 설사, 
  • 염증성 설사, 
  • 혼합형 설사 등으로 설사를 분류할 수 있다. 

설사의 원인

  1. 바이러스 위장염-로타바이러스, 에코바이러스, 엔테로바이러스, 콕사키바이러스, 폴리로바이러스 등 장바이러스, 놀와크바이러스, 장아데노바이러스, 애스트로바이러스, 케리시바이러스, 코로나바이러스, 또는 토로바이러스 등 여러 종류의 바이러스 중 한 종류의 바이러스가 위장에 감염되어 바이러스 위장염을 일으킬 수 있다. 그로 인해 설사를 할 수 있다.
  2. 박테리아 위장염-살모넬라균, 시겔라균, 캄필로박터균, 황색 포도상구균, 비브리오균, 클로스트리듐균, 예르시니아균, 대장균 등이 위장에 감염되어 박테리아 위장염을 일으키고 그로 인해 설사할 수 있다.
  3. 람불편모충(Giardia lamblia) 감염으로 위장염이 생길 수 있고 그로 인해 설사할 수 있다 
  4. 선모충(트리키넬라)기생충 감염으로 장염이 생길 수 있고 그로 인해 설사할 수 있다.
  5. 신경질적인 아이들이나, 자율신경이 예민한 아이들은 정신적 불안, 주위 환경 변화로 인한 불안 등으로 위장 연동운동 이 항진되어 그로 인해 설사할 수 있다.
  6. 체질에 맞지 않는 음식물을 먹거나 과식을 할 때 설사할 수 있다,
  7. 감기를 비롯한 여러 가지 바이러스 상기도염이나 중이염 등 소화계통 이외 신체 다른 계통의 장기에 생긴 감염병을 앓을 때도 설사할 수 있다.
  8. 출생 이후부터 28일까지 기간을 신생아기라고 한다. 신생아들에게 생기는 설사의 원인과 신생아기 이후 영유아들이나 학령기 아이들에게 생기는 설사의 원인은 다소 차이가 날 수 있다. 신생아기 이후 영유아들이나 그 이후 아이들에게 생길 수 있는 설사의 원인을 다른 제목 하에 설명한다.

Diarrhea Mechanism

1. When the osmotic pressure of the intestinal fluid in the intestinal tract (intestinal) is higher than normal, it passes through the intestinal wall mucosal layer and secretes too much intravascular fluid and tissue fluid into the intestinal tract. say

2. Gastrointestinal hyperactive diarrhea, which may be caused by excessive intestinal peristalsis, such as in the presence of excitatory gastrointestinal syndrome

3. Viral gastroenteritis or bacterial gastroenteritis can damage the intestinal wall mucosal layer, resulting in an abnormally large amount of fluid in the blood being secreted into the intestinal tract. Diarrhea that occurs when the intestinal fluid in the intestinal tract is not normally absorbed into the blood vessels of the intestinal mucosa is called secretory diarrhea.

4. Inflammatory bowel disease such as ulcerative colitis or Crohn’s disease causes inflammation in the mucosal layer of the intestinal wall. As a result, the intestinal fluid increases and the blood in the blood is secreted more than normal into the intestinal tract, resulting in diarrhea.

5. As described above, theoretically, one type of diarrhea can occur due to the mechanism of each type of diarrhea, but in most cases, it is common to have diarrhea due to several mechanisms occurring at the same time. Diarrhea that does this is called mixed diarrhea.

• As already explained, in theory, depending on the mechanism by which diarrhea occurs,

• osmotic diarrhea;

• secretory diarrhea;

• intestinal peristaltic hyperdiarrhoea,

• inflammatory diarrhea;

• Diarrhea can be classified as mixed diarrhea.

Causes of Diarrhea

1. Viral gastroenteritis-Rotavirus, echovirus, enterovirus, coxsackievirus, poliovirus, etc. enterovirus, Norwalk virus, enterovirus, astrovirus, calicivirus, coronavirus, or torovirus One of the viruses in the gastrointestinal tract can infect the stomach and cause viral gastroenteritis. This can cause diarrhea.

2. Bacterial gastroenteritis – Salmonella, Shigella, Campylobacter, Staphylococcus aureus, Vibrio, Clostridium, Yersinia, Escherichia coli, etc. can infect the stomach and cause bacterial gastroenteritis, resulting in diarrhea.

3. Giardia lamblia infection can cause gastroenteritis and diarrhea

4. Trichinella (trichinella) parasite infection can cause enteritis, which can lead to diarrhea.

5. Nervous children or children with sensitive autonomic nerves may have diarrhea due to increased gastrointestinal peristalsis due to mental anxiety or anxiety due to changes in the surrounding environment.

6. You may have diarrhea when you eat food that is not suitable for your constitution or when you overeat.

7. Various viruses including colds You may have diarrhea when you have an infectious disease that occurs in organs other than the digestive system, such as upper respiratory tract infection or otitis media.

8. The period from birth to 28 days is called a neonatal period. The causes of diarrhea in newborns and the causes of diarrhea in infants and school-age children after the neonatal period may be somewhat different.
